Submit a birth certificate, certificate of immunizations (Form 121), or school registration documents containing the parent's name for each child who needs childcare.
All children in the household must be cooperating with child support. Submit a divorce decree, court order, or a Mississippi Verification for Child Support Services form.
If you are not the biological parents of a child who needs care, you must submit a court order or a Guardianship/In Loco Parents Verification Form ( you can download this form at: https://www.mdhs.ms.gov/wp-content/uploads/2021/03/In-Loco-Form-1.pdf
Submit (1) one form of verfication from the following list: a copy of the parent's current driver's license, state issued ID, passport, or passport card OR Submit (2) forms of verifications from the following list: work identification, school identification, health benefits identification, voter registration card, parents'/guardian birth certificate, or EBT card.
Submit a current Mississippi driver's license, Mississippi state issued identification, bank statement, cell phone bill, utility bill, cable bill, rent or lease agreement, mortage document, or employment records.
Check stubs: Submit a copy of all paychecks stubs issues within the past thirty days to verify at least 25. This information is required for the applicant, spouse, and any biological parent living in the home.
If you have new employment and have not received your first paycheck, you must submit a letter from your employer on company letterhead with the following information: start date of employment, rate of pay, and work schedule including hours worked. This information is required for the applicant, spouse, and any biological parent living in the home.
Check: If you are paid by personal check, you must submit copies of the front and back of two checks that have cleared the bank. This information is required for the applicant, spouse, and any biological parent living in the home.
Cash: If you are paid in cash, you must submit a notarized letter from your employers on company letterhead. This letter must contain the following information: start date of employment, income, rate of pay, and work schedule including hours worked. This information if required for the applicant, spouse, and any biological parent living in the home.
